Medications

The instructions in this document are necessary for safe administration of the medications and following them is required by the camp.

If your child needs to be any administered any prescription or OTC medications on a regular or need basis, they must be provided to an adult in charge of these medications.  The Scout can not keep any medications with him.

Forms:

• 1. This form must be filled out for each medication (Rx or OTC - one per medication)

  2. This form must be filled out once combined for all medication (even if there is just one medication, you need to fill out this besides #1).

  3. The medications must be listed in the Annual Health and Medical Record (Part B).

Packing and other Instructions -

• 1. Medications must be in their original containers with the label.

  2. Place each medication with its form (#1) in a small Ziploc bag.

  3. Place all small Ziploc bags in a large 1 gallon size Ziploc bag along with Form #2

  4. Write down you Scout's full name on the large bag.

  5. ATTENTION: Medications not packed, not in the original containers or not accompanied by the forms can not be accepted.  

  6. Mr. Gulati and Mrs. Smith will collect all medications from a parent on the day of the departure unless you can bring them on Tuesday 7/11 at the meeting (again, Scouts should not bring the medications).  

  7. Please talk to Mrs Smith or Mr. Gulati if your medication needs refrigeration.

  8. Unused medications must be picked up by the parent on the day we return from the camp.
