Permission Slips

Your son will need various types of permission slips depending on the activity/outing.  The most common is the Troop permission slip.  The others could be firearms permission slip, or permission slips from other camps.

• • Troop 125 permission slip: There are two copies on one sheet.  Please keep the lower one for next time!

  • Combined Troop 125 permission slip and SFBAC firearms permission slip: Use this for shooting sports day/camp and Wente summer camp 

    • If you do not want your son to participate in any shooting sports at Wente, please fill out only the Troop permission slip
