No medical exam is needed for Parts A/B
Please understand why you are filling this out - it is not because the Troop wants it. It is because a doctor will need the information to treat your son well in case of any need. Please provide all the information in a legible way (computer filled if possible). It is your responsibility to make sure this form is current throughout the year.

Download this form, and save locally on the hard disk. Please note that older versions are not accepted by BSA.
There are a total of three pages (for Parts A and B) which you fill out on computer.
Do not fill it from within the browser. You will not be able to save the information you fill. Please use Acrobat Reader to fill the information once you have it saved locally on your hard disk.
After entering some information, save, close and reopen to make sure your data is saved. Then complete rest of the form.
Check off the boxes in the checklist you printed as you fill the form.
Once done, print/copy the three pages DOUBLE SIDED. If printing double sided manually, please refer to the checklist for the order of pages.
Now make copy of BOTH sides of the insurance card on side 2 of second sheet. Yes, it is a hassle, but just once a year, and it saves an adult leader carrying 100 extra sheets to every outing. as well as shredding when you fill out a new form.
No need of the back side if the insurance company's phone number is on the front side.
It is possible your health insurance company allows you to print a card from your account. This will make it easier to put both the sides of the card on single sheet.
Sign it (by hand, no digital signature please):
(Adult form only) Participant on Part A
(Scout form only) Both parents on Part A (California law)
(Scout form only) Part B2 - about half way down, if you are authorizing administration of non-prescription medications
Attach the checked off checklist to the front of Part A
Give it to the Committee Chair or Scoutmaster.
This form is filled out when a Scout joins the Troop, as well as annually in Fall. Please remember where you stored the form on your computer - it may save you typing next time.
