Parents/Guardians - required for at least one:
Name, cell phone number, and e-mail address
Parents/Guardians - needed only for BSA membership and/or joining an activity

Medical form parts A/B/C (Scout/Adult): paper copy (refer to this page for exact instructions)
Copy of health insurance card is part of Part B
Phone number, street address, food preferences: e-mail to Committee Chair
E-mail address: Send an e-mail from that address, or forward a mail you received from that address WITHOUT removing the headers. E-mail addresses are copied (not hand typed) to prevent typos.
YPT certificate: soft copy preferred (shared through Google Drive or e-mail to Committee Chair) or printed copy
Driver's license and proof of auto liability insurance: soft copy preferred (shared through Google Drive or e-mail to Committee Chair) or printed copy
